Tesamorelin is a chemically modified growth hormone-releasing hormone (GHRH) analog composed of 44 amino acids. It is a trans-3-hexanoic acid version of natural GHRH, where the trans-3-hexanoic acid group is added to the N-terminus while the C-terminus is amidated and acetylated. This structural modification may increase its stability and extend its half-life compared to endogenous GHRH. Tesamorelin appears to mediate the positive influence of GHRH and other GHRH analogs such as GRF(1-29), CJC-1295, and Sermorelin by maintaining the physiological activity of GHRH without disrupting the natural pulsatile rhythm of growth hormone release. Research suggests that Tesamorelin may potentially stimulate growth hormone secretion from anterior pituitary somatotrophs through GHRH receptor binding, which initiates intracellular signaling cascades involving G-protein activation, cAMP production, and downstream gene transcription associated with growth hormone synthesis.
